WD, LLC would like to welcome you to our practice. Attached you will find a Patient Information Sheet and Patient Medical History Sheet. Please fill out the front and back of these forms and bring to your appointment. If the appointment is for a minor child, please have the child’s social security number available. In addition, a list of all your medications that you are presently taking is needed. You will also need your insurance card, driver’s license and any co-pay or deductible that will you be responsible for at the time of service. Co-pays and deductibles will be collected upon your check-out. For your convenience, we accept cash, most credit cards and Care Credit.

If you are not able to keep your appointment, you must notify us within 24 hours. You may be charged a fee if proper notification is not given. If you have any questions, please feel free to call us at 843.449.0453.
